Privacy Policy – New Stanford Hospital Opening App

This Privacy Policy explains how Stanford Health Care (“SHC” or “we”) collect, use, share and protect information in relation to our mobile services, web site, and any software provided on or in connection with the New Stanford Hospital Opening App (“NSHO APP”) and website services (collectively, the “Service”), and your choices about the collection and use of your information.

Acceptance of Terms and Conditions
When you access, use or view the Service, you agree to be bound by the terms and conditions of this Privacy Policy (“Terms and Conditions”). Please note that SHC may update these Terms and Conditions at any time by making the update available on the NSHO APP or the Service. If you continue to access, use or view the Service after such updates have been made, you consent to the updated Terms and Conditions and you will be bound by them; therefore, we advise you to periodically review this page for such revisions. If you do not agree to these Terms and Conditions, please discontinue use of the Service promptly. The Service and all content are also subject to the SHC website’s general terms and conditions.

Our Privacy Policy applies to all visitors, users, and others who access the Service (“Users”)

1. INFORMATION WE COLLECT
We collect the following types of information.

The NSHO APP collects Personal Information in order to provide you with a positive experience. The Personal Information refers to any information that you voluntarily submit and consent to us and that identifies you personally. Some data will be collected automatically by the NSHO APP to track your usage.

By submitting any contact or subscription form on our website or the NSHO APP, you agree to receive messages from us. You will always be free to easily unsubscribe at any time following our opt-out instructions. If you do not want to receive our messages and/or communication, please do not submit any form on the NSHO APP.

SHC may import your data to our platform to send App invitations to download or use the NSHO APP.

For a better experience while using the NSHO APP, you may be required to provide us with certain personally identifiable information, including but not limited to android.permission.GET_ACCOUNTS, android.permission.READ_CONTACTS, android.permission.READ_PHONE_STATE. The information that We request will be retained by us and used as described below.

Upon accessing the NSHO APP, our web server collects and stores some or all of the following usage data: (1) the domain name from which you access the Internet; (2) the date and time you access the Service; (3) the pages and screens you visit; (4) the address of the website from which you linked directly to us; (5) the name and release number of the web browser software, device details and operating system you are using; (6) the IP (Internet Protocol) address of the mobile device or computer you are using; (7) First name, Last name, Email Address, and Password you submitted while registering for an account on the App; and (8) Your usage and activities on the App including any rewards points you may collect on the Service. This information is used by SHC for internal evaluation, quality control, and other reasons deemed appropriate by SHC. Your continued use of the Service will be taken as consent to the collection, use and disclosure of the usage data described above.

The NSHO APP does use third party services that may collect information used to identify you:

We may collect some or all of the following data:

  • First Name
  • Last Name
  • Email Address
  • Profile Picture
  • IP Address
  • Device details
  • Social profiles data that you share with us

TrueDepth API:
The NSHO APP uses the TrueDepth API whenever the user chooses to utilize the “Take A Snapshot” feature of the App. While we do allow the user to store and share their snapshot, we are not collecting, storing or sharing any TrueDepth API data itself during this process.

When we collect information from the debug log, we are collecting the following information:

  • API requests, responses, date, time & errors
  • App screen name while opening screen
  • User activity on the App, and tap actions
  • Device information such as Device Manufacturer, Model, OS, OS Version, Time zone, Device unique key & IP address)
  • User information such as first name, last name, email address and profile picture.

Information Sent by Your Mobile Device:
We collect certain information that your mobile device sends when you use the NSHO APP, like a device identifier, user settings and the operating system of your device, as well as information about your use of the Service.

Location Information:
When you use the NSHO APP on your mobile device, we may collect and store information about your location by converting your IP address into a rough geo-location or by accessing your mobile device’s GPS coordinates or by proximity to a Bluetooth beacon or course location if you enable location services on your device. We use location information to provide SHC relevant location services such as navigation at our facilities. If you do not want us to collect location information, you may disable that feature on your mobile device, but you will not be able to utilize location enabled services of the NSHO APP.

Cookies and similar technologies:
When you visit the NSHO APP, we may use cookies and similar technologies like pixels, web beacons, and local storage to collect information about how you use the NSHO APP and provide features to you.

Log file information:
Log file information is automatically reported by your browser each time you make a request to access (i.e., visit) a web page or app. It can also be provided when the content of the webpage or app is downloaded to your browser or device.

When you use our Service, our servers automatically record certain log file information, including your web request, Internet Protocol (“IP”) address, browser type, referring / exit pages and URLs, number of clicks and how you interact with links on the Service, domain names, landing pages, pages viewed, and other such information. We may also collect similar information from emails sent to our Users which then help us track which emails are opened and which links are clicked by recipients. The information allows for more accurate reporting and improvement of the Service.

Device identifiers:
When you use a mobile device like a tablet or phone to access our Service, we may access, collect, monitor, store on your device, and/or remotely store one or more “device identifiers.” Device identifiers are small data files or similar data structures stored on or associated with your mobile device, which uniquely identify your mobile device. A device identifier may be data stored in connection with the device hardware, data stored in connection with the device’s operating system or other software, or data sent to the device by NSHO APP.

A device identifier may deliver information to us or to a third-party partner about how you browse and use the Service and may help us or others provide reports or personalized content. Some features of the Service may not function properly if use or availability of device identifiers is impaired or disabled.

Metadata:
Metadata is usually technical data that is associated with User Content. For example, Metadata can describe how, when and by whom a piece of User Content was collected and how that content is formatted.

2. Passwords and Security

Service Enrollment
Your enrollment in the Service is contingent on verification of your identity via email. After you create your account with your Email Address and password, you will receive a confirmation email from us to select a password. Once you have completed this step, you will be required to use this information to sign in to the NSHO APP.

Passwords
The security of your information contained in the Service is dependent upon selecting a secure password. It is your responsibility to create a secure password. At a minimum your password must be between eight and fourteen characters long, contain at least one letter and one number and cannot be the same as your User ID. Passwords should not contain words, your birthday, your social security number or any other easily guessed information.

It is your responsibility to safeguard your User ID and password, and to change your password if you believe the security of your information has been compromised. In addition, in order to maintain the security of the site, you may be forced to change your password at any time, without prior notice, upon login and before you are allowed access to your account.

You can change your password by clicking the “Change Password” link on the NSHO APP My Profile screen.

3. HOW WE USE YOUR INFORMATION
In addition to some of the specific uses of information we describe in this Privacy Policy, we may use information that we receive to:

  • provide personalized content and information to you and others, which could include marketing
  • provide, improve, test, and monitor the effectiveness of our Service
  • develop and test new products and features
  • monitor metrics such as total number of visitors, traffic, and demographic patterns
  • diagnose or fix technology problems
  • automatically update the NSHO APP application on your device

Email Opt-Out
SHC may periodically send you emails with information related to health services, wellness, events and news about Stanford Health Care. You can unsubscribe from these emails by clicking on the “unsubscribe” link in the bottom of the email.

3. SHARING OF YOUR INFORMATION
We will not rent or sell your information to third parties outside SHC without your consent, except as Parties with whom we may share your information: in this Policy.

Parties with whom we may share your information:
We may share your information as well as information from tools like cookies, log files, and device identifiers and location data, with third-party organizations that help us provide the Service to you (“Service Providers”). Our Service Providers will be given access to your information as is reasonably necessary to provide the Service under reasonable confidentiality terms.

We may remove parts of data that can identify you and share anonymized data with other parties.

We may also combine your information with other information in a way that it is no longer associated with you and share that aggregated information.

4. Links to Third Party Sites
This Service may contain links to other Internet sites (“Third-Party Sites”) that are not owned or maintained by SHC, and may also contain content provided by our licensees. Links to Third-Party Sites and information provided by our licensees are included for your convenience only and do not imply approval of or endorsement by SHC of those sites or such content. SHC makes no warranties or representations and disclaims any responsibilities for the content of and products or services offered by Third-Party Sites. Further, SHC makes no warranties and disclaims any responsibility for the content, products, or services offered or provided by our licensees and contained in the Service.

5. Sharing Data with 3rd Party Apps
NSHO APP allows users to interact with 3rd Party Apps using a standard set of application programming interfaces (APIs).

Before granting access, please read the 3rd party app’s terms and conditions and privacy policy carefully to be sure that you understand how the 3rd party app and app developer may use your data and information you share.

The 3rd party app was not created by Stanford Health Care and Stanford Health Care accepts no liability for the 3rd party app. Stanford Health Care is not an agent for 3rd party app developers nor does Stanford Health Care endorse or guarantee 3rd party apps. Stanford Health Care makes no representation or warranty regarding the accuracy or suitability of the 3rd party app.

By granting access, you are allowing the 3rd party app to access your data and information you share. Stanford Health Care has no control over the 3rd party app or your data and information, including how the 3rd party app further discloses your data and information, once your data and information has been accessed by the 3rd party app. Note that Stanford Health Care’s privacy policy does not extend to the 3rd party app. The 3rd party app developer may not be subject to the same obligations to protect your data and information as Stanford Health Care, and your data and information might be used for advertising or other secondary purposes.

7. CHANGES TO OUR PRIVACY POLICY

Your continued use of the Service after any modification to this Privacy Policy will constitute your acceptance of such modification.
